The Annual Chapter Report Form for 2024, and future years, is found under the “Leadership Conference” menu. Or click HERE for the form. The report to submit for the upcoming Leadership Conference awards will summarize chapter activities and information for the year 2024. It is a Word document, so you will need the Microsoft Office Word program to open and complete the form.
